- Unit Notes:OSF ITEMS OF NOTE: - Local Rate: Applies to clinicians with permanent addresses within 50 miles (-$25 to regular and OT bill rates) - OSF Call Off Policy: Equivalent of 12 hours every 13 weeks with no penalty or need to pay for the shift - Non-billable Orientation Hours: 12 hours, guaranteed hours will not apply during orientation REQUIREMENTS FOR SUBMISSION: - Last four (4) digits of clinician's SSN - Clinician's middle initial - Requested time off - Active IL license or confirmation that application process has been initiated - Your candidate understands OSF is a Drug-Free Campus and they must produce a negative drug screen for all panels Department: Hospice | RN - Required Certifications: BLS. Valid driver's license & insurance. RN license in IL. - Common diagnosis /Types of patients/patient demographics: End of life care/Hospice or Home Health. - RT Equipment (vents, Cpap, BiPap, etc): No vents. - Experience or 'Must have' skills: Independent worker, works remotely, computer/EMR skills. - How many patients do they see per day?
