- Key Requirements Summary: 1. Experience: Minimum 1 year required; must be recent; hospice experience required; OASIS experience required for staff completing SOC (Start of Care); no first-time travelers accepted 2. Cases: Geriatric end-of-life care; average 5-7 patients per day (confirm during interview); 30 points weekly minimum 3. Certifications: BLS required 4. License: License in hand required at time of submission 5. Education: Not specified 6. EMR: HCHB (Homecare Homebase) proficiency required 7. Shifts/Call: 8am-5pm; on-call rotation may be required; discuss further during interview; guaranteed 40 hours 8. RTO: Must be submitted at time of application 9. Returning Staff: Former permanent employees must be separated for a minimum of 6 months before returning as a traveler; must be approved by HR 10. Travel/Local: Territory up to 60-mile radius from hub/office; locals accepted at travel rate; traveler expected to be flexible with territory; active driver's license and personal transportation required; travel pairs accepted but each assigned own territory and must have own transportation; mileage reimbursed at IRS standard rate 11.
