- Key Requirements Summary: 1. Experience: Minimum 1 year Hospice experience required; 2 years overall as a clinician required 2. Cases: In-home Patient Care, Interdisciplinary Care Coordination, Plan of Care Development and Oversight, Geriatric Population 3. Certifications: BLS required; valid RN license in hand at time of submission; pending license not accepted 4. License: Valid state RN license required in hand at time of submission 5. Education: Not specified 6. EMR: HCHB (Homecare Homebase) and Oasis required; 1 year experience with both preferred; charting must be completed while at patient home 7. Shifts/Call: On-call rotation required; must be open to 1 night of call during the week (discussed upon interview); weekends as needed; 30 points per week required for full-time status (32 hours guaranteed); OT must be pre-approved 8. RTO: Not specified 9. Returning Staff: Not specified 10. Travel/Local: Must be willing to travel up to 50 miles outside home base; locals accepted within 50-mile radius; mileage reimbursed at IRS standard rate 11. Background: Not specified 12.
