- Key Requirements Summary: 1. Experience: 1 year Home Health experience required; greater than 3 months OASIS documentation experience required 2. Cases: Start of Care, Recertification, Resumption of Care, Significant Change in Condition, Routine Visits, Transfer, Discharge; Medicare patient starts of care frequency must be included with submission 3. Certifications: BLS (AHA) required 4. License: Hawaii (HI) RN License required 5. Education: Not specified 6. EMR: HomeCare HomeBase preferred; HCHB documentation preferred 7. Shifts/Call: Days, 5x8s; 25 units productivity per week; no on call 8. RTO: One pre-approved time off 9. Returning Staff: Not specified 10.
