- Key Requirements Summary: 1. Experience: 1 year of Home Health experience required; greater than 3 months of OASIS documentation experience required 2. Cases: Adult Home Health; Start of Care; Admission Assessment; Recertification; Resumption of Care; Routine Visit; Significant Change in Condition; Transfer; Discharge; Medicare patient starts of care (frequency per week must be included with submission) 3. Certifications: BLS (AHA) required; HI License required 4. License: Hawaii (HI) RN License required 5. Education: Not specified 6. EMR: Charting system experience required; OASIS required; HCHB documentation preferred; MatrixCare preferred 7. Shifts/Call: Days, 5x8s; 25 units productivity per week; no on-call 8. RTO: One pre-approved time off noted 9. Returning Staff: Not specified 10. Travel/Local: Floating to cover Oahu, Maui, and/or Kauai may occasionally be required; all float expenses paid 11.
