- Key Requirements Summary: 1. Experience: 1 year minimum, recent experience required 2. Cases: Geriatric end of life care; OASIS experience required for staff completing SOC (Start of Care) - Home Health only 3. Certifications: BLS required 4. License: License in-hand required with submission 5. Education: Not specified 6. EMR: HCHB (Homecare Homebase) experience required 7. Shifts/Call: 4 10s, Tues-Friday; on-call rotation may be required - discuss during interview 8. RTO: Must be submitted at time of application 9. Returning Staff: Former permanent employees must be separated minimum 6 months before eligible to return as traveler; must be approved by HR 10. Travel/Local: Up to 60-mile radius from hub/office; locals accepted at travel rate 11. Background: Not specified 12.
