- Unit Notes:Start date goal: 11/26/23 Shift: M-F 8-4:30pm - 40 guarantee Call requirements: Every 4th weekend (flex a day from the M-F shift) No required certification # years experience: 1 year of home care or hospice experience. Prefer experience with Meditech home care EMR. Anything else? They will need a reliable car, valid driver's license, and auto insurance. Required Certifications: BLS, Valid Driver's License & Insurance, must be licensed and fingerprinted in CT. Common diagnosis / Types of patients/patient demographics: Adults and geriatrics, No pediatrics - All common patients, medication management, CFH, COPD, post surgical, Ortho - total joints, CVA, dementia, hospice. Additional Job Duties/Utilization Review involved?: Evaluations, treatment, plan of care. RT Equipment (vents, Cpap, BiPap, etc): No vents, o2 only Experience or 'Must have' skills: 1 year experience, highly preferred Oasis experience. How many patients do they see per day?: Weekly point system- start of care is 3points, general visit are 1point. On average 5-6 visits a day. Manager/Supervisor: Administrator available 24/7, Manager M-F Home Health/Hospice Aids?: Yes Shifts & Scheduling (Do you allow self-scheduling? Block scheduling?): Self scheduling M-F 8-4:30, weekend rotation Weekend Requirements: Covered by Call - Possibly full-time weekend position available. Holiday Requirements: Rotate in with perm staff Is there a call or standby requirement?: Weekend call. Callback to client within 20 min and visit within 30 minutes on weekends. Travel Requirements: Typically, within a 20-minute radius, urban area Scrub Color/Dress Code: Any color as long as they are clean and professional Unit/Department specific orientation: 1 week with preceptor Cancellation Policy:Client may cancel up to three (3) shifts in a thirteen (13) week assignment without penalty Charting System:Oasis (Home Health) BSN Required:False License to Submit:Yes Trauma Level: Care Setting:Post Acute Division:Nursing Shift Notes:8am-4:30pm Monday-Friday with some overnight on call responsibilities Required Certifications:BLS Certification
