- Unit Notes:No clinical involvement - profiles go directly to the manager for consideration **MUST HAVE: HOME CARE HOME BASE EXPERIENCE AND BACKGROUND IN HOME HEALTH - NO EXCEPTIONS Coverage area: Nelson, Spencer and southern Bullett counties - (Mileage not billable) Required Certifications: None in addition to a valid Driver's License & Insurance - do need hospice experience though. Common Diagnoses / Types of Patients / Patient Demographics: Pediatrics and adults Additional Job Duties / Utilization Review Involved? No Do you use a point system? How many points per day/week? Do you require travelers to complete Start of Care, recerts, and daily visits? Start of Care, recerts, and daily visits. RT Equipment (Vents, CPAP, BiPAP, etc.): No Experience or "Must Have" Skills: Hospice and HCHB experience How Many Patients Do They See Per Day? No point system. Manager/Supervisor Availability:On call as listed below. Home Health/Hospice Aides? Yes Shifts & Scheduling: Monday Friday, 8:00 AM 5:00 PM. Clinicians are expected to call patients to schedule visit times. Weekend Requirements: Yes, every 3rd weekend. Holiday Requirements: No. Is There a Call or Standby Requirement? Yes. One weeknight and every 3rd weekend (scheduled and on-call visits). Floating Requirements: No Scrub Color / Dress Code: Any color scrubs. What is their availability after hours/weekends/holidays? On call as listed above.
