- Key Requirements Summary: 1. Experience: 2 years acute inpatient RN Case Management experience required 2. Cases: Adult ED and Adult inpatient units experience required; discharge planning including transitional care, home health, SNF, inpatient rehab; behavioral health case collaboration required; knowledge of CMS and CoP helpful 3. Certifications: BLS required; ACLS and PALS required for certain departments (OR, Ambulatory Surgery, Pediatrics/PICU, PACU, GI Lab) 4. License: Current RN licensure permitting work in the State of Arizona required 5. Education: Graduation from a qualified, nationally-accredited nursing program required 6. EMR: Epic experience required; additional software includes Microsoft Teams, Outlook, Word/Excel/PowerPoint, Health Stream, ACMA, Strata Health, UKG 7. Shifts/Call: Monday-Friday 0800-1630; weekend coverage once every 4 weeks with a compensatory weekday off; minimum 3 weekend days (Saturday or Sunday) required during the 13-week contract 8.
