Epic Travel Staffing is hiring a Local RN – Case Manager

Shift: Days, 8a-4:30p, 8×5, 40hrs per week

Duration: 13 weeks

Every other weekend required

Requirements:

• CA RN License
• 5+ years of RN Case Management experience (required)
• Must have Inpatient Acute Case Management (Hospital Case Manager) experience
• Previous Charge experience preferred
• BLS

Other Details:

• Skills required: Care Coordination, Discharge Planning, Utilization Review, Acute Hospital experience
• Patient population: Adults, Geriatrics
• Patient ratios: Days 1:15, Weekends 1:25
• Responsible for Care Coordination and Care Transitions Planning throughout the acute care patient experience
• Works in collaboration with Physician, Utilization Manager, Medical Social Worker, and bedside RN to assure timely progression and transition of patients
• Locals accepted within 50-mile radius
