Requirements

- Active California RN license required before start
- Key Requirements Summary: 1. Experience: 3 years of case management experience required; recent case management experience required 2. Cases: Pilot project supporting liver failure patients across ED, OBS, and inpatient settings; working closely with multidisciplinary team and CHW for care navigation and discharge planning 3. Certifications: BLS required; BSN required; must be AHA or American Red Cross (implied by BLS requirement) 4. License: Active CA RN License required; no temporary licenses accepted 5. Education: BSN required 6. EMR: Epic 7. Shifts/Call: 5x8s or 4x10s accepted; no on-call required; no weekend requirement; no floating required 8. RTO: Not specified 9.
