Experience: Minimum 5 years of recent acute care Case Management experience in a hospital setting; less than a 6-month gap since last acute care Case Management role; previous patient census of approximately 15-25 patients; rated 4/5 or 5/5 across all Case Management functions including knowledge of CMS regulations 2. Cases: Adult acute care hospital setting with discharge planning; Utilization Review (UR) experience required; experience in a Level I, II, or III acute care hospital 3. Certifications: BLS through the American Heart Association (required); CCM - Certified Case Manager (preferred); ACM - Accredited Case Manager (preferred) 4. License: Active Virginia or Compact RN license in hand at time of submission; no exceptions; current NURSYS report required at submission 5. Education: Not specified 6. EMR: Epic proficiency required; no exceptions 7. Shifts/Call: 5x8 Days, 8am-4:30pm; weekdays with some weekends and holidays; no overtime without supervisor approval; travelers must work two of the three winter holidays; 5-6 weekend shifts per 16-week contract 8.
