- Active California RN license required before start
- Unit Notes:sent to manager for interview need 2 sup refs (charge accepted) at submittal; 1 w/in 12 mo. & 1 w/in 3 years worked Candidates may not have been directly employed (full time, part time, or PRN) by any facility associated with CommonSpirit, including CHI and/or Dignity within the past year. Candidates who were employed PRN through outside firms will be considered. *****MUST HAVE PRIOR RECENT CA DIGNITY EXPERIENCE OR THEY WILL NOT CONSIDERED*** Unit Case Management/Care Coordination # patients seen/consults per day 15-20 Certs: BLS EMR: Interqual Weekend Requirement every other depending on staffing. Would be at least one full weekend per month Holidays not required, but will work if they are willing Scrub Color/Uniform business causal attire Hospitalist or Intensivist in house yes RT 24/7 yes Pharmacy 24/7 no Shifts depends on dept but hours are between 8-5pm Do they cover all areas of hospital? Any outpatient? Any home care? All areas of the hospital, no OP and no Home Care Floor specific orientation: 5 days of care coordination new employee orientation, unit orientation timeframe depends case by case Common duties/ Required skills or knowledge: Dealing with all payers, uninsured, homeless. Completing Initial assessments, IQ, DC planning, cerner Additional Notes Is this position UR or discharge planning or both? Both Additional Questions Do you require case managers to be familiar with Interqual criteria? yes What other specific skills or knowledge are needed to be successful as a case manager at your facility? collaboration, teamwork, effective communication, ability to reprioritize throughout the day. Are they required to do Utilization review as well? yes We will train candidates in IQ. Both DC Planning and UM are required. Cancellation Policy:Hospital can cancel up to 4 shifts per 13-week assignment, 3 shifts for assignments that are less than 13 weeks Charting System:Cerner BSN Required:True License to Submit:Yes Trauma Level:2 Care Setting:Acute Division:Nursing Shift Notes:8hr Days, EOW, First to Float Required Certifications:BLS Certification
