- Key Requirements Summary: 1. Experience: 3 years required; will accept first-time traveler with inpatient hospital/acute care experience 2. Cases: 15-20 daily census; inpatient case management providing and coordinating medical home care, placement, and social issues; wrap-around services for safe discharges; SDOH screening, case management assessments, care coordination, and referrals to outpatient and post-acute resources 3. Certifications: BLS preferred; must be noted as preferred, not required 4. License: RN MD state license required 5. Education: Not specified 6. EMR: Epic experience required; preferred noted in narrative 7. Shifts/Call: 5x8 Days, Monday through Friday block (08:00 or 08:30-16:00 or 16:30 depending on needs); occasional weekend staffing on a case-by-case basis; holidays required as they fall per schedule 8. RTO: No pre-approved time off 9. Returning Staff: Not specified 10. Travel/Local: Float required between Bel Air and Aberdeen facilities as needed (10-mile radius between facilities) 11.
