Cases: Acute Care Case Management and Discharge Planning; experience required with assessing patients, care coordination, utilization review, HMOs/IPAs, DME, SNF, ambulance transport; knowledge of adoption/foster care, drug/alcohol abuse treatment, homelessness, mental health, advance directive, POLST, protective services 3. Certifications: Not specified 4. License: CA RN License required; pending license accepted but must be licensed by start date 5. Education: Not specified 6. EMR: Epic 7.
