- Key Requirements Summary: 1. Experience: 2 years Acute Care Case Management/Discharge Planning required; must have hospital experience; behavioral health experience does not apply 2. Cases: Acute Care Case Management; experience required with assessing patients, care coordination, discharge planning, and utilization review; experience with HMOs, IPAs, etc.; knowledge of adoption/foster care, drug/alcohol abuse treatment options, homelessness, mental health, advance directive, POLST, and protective services; competent with arranging DME, SNF, and ambulance transport independently 3. Certifications: Not specified 4. License: CA state license required; pending license accepted but must be active by start date 5. Education: Not specified 6. EMR: Epic required 7.
