- Key Requirements Summary: 1. Experience: 1 year required for submission 2. Cases: LTC experience required; LTAC experience preferred; patient diagnoses include Dementia, Alzheimer's, TBI, Plegics, Cognitive Impairments, Physical Impairments, and other LTC-level appropriate diagnoses 3. Certifications: BLS required; AK license MUST be in hand at time of submission; Alaska Food Worker Certification required (obtained during on-site orientation; one-time $10 fee); all certifications must be AHA or
