- Key Requirements Summary: 1. Experience: 2 years required (hard stop); seasoned traveler in blood bank required; recent/current blood bank experience within 2 years; large hospital experience (300+ beds) required 2. Cases: Blood bank experience absolutely required; high volume type/screen and crossmatch; 30-50 samples per day; trauma cases 3. Certifications: ASCP-BB required; Clinical Immunohematology Science preferred; LA fire card within last 30 days required 4. License: CA license must be in hand 5. Education: Not specified 6. EMR: Epic experience required (hard stop); Epic Beaker experience required 7. Shifts/Call: 4x10 hour variable days; start time 2130 but may flex to 2000; work fully independent at night; weekends may exceed every other; no on-call required 8. RTO: Maximum 7 days (Aya and Client offer); must commit to working 3 of 5 holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day); RTO must be included at time of submission 9. Returning Staff: Can return after 3 months; must disclose previous Providence facility employment with location and dates 10. Travel/Local: 50-mile radius rule; locals accepted at lower rate; must be seasoned traveler; first-timers not accepted 11. Background: Must disclose license/background hits upfront at submission; no open license hits approved 12. Modules: 16 hours average, non-billable; completed pre-start and annually; over 20 hours requires PM approval; self-reported completion via Workramp transcripts 13. Travel Experience: Required; seasoned blood bank traveler required; multiple travel assignments in blood bank required 14. Scrubs: Royal Blue for RNs; L&D/OR scrubs provided 15. Flu Vaccine Requirements: Unknown 16. Special Equipment: Wellsky and Immucor Instrumentation preferred; blood bank software experience required; Epic Beaker Critical: Trauma Level I experience required; teaching hospital preferred; must work alone with confidence; 2 references covering 1 year within last 3 years required; all travelers expected to float between facilities within state.
