- Key Requirements Summary: 1. Experience: 2 years required; discharge planning experience including admission criteria review, benefits eligibility, care coordination, conducting needs assessments and order DME; experience with home health, hospice placement, LTAC, nursing home placement, rehab placement, Acute hospital experience 2. Cases: Geriatrics, ICU, Heart and Vascular, General Surgery, General Medicine, MICU 3. Certifications: BLS required; will accept RN Case Manager/Discharge Planner or Licensed Social Worker (LMSW/LGSW/LCSW - must have taken and passed exam) 4. License: DC RN Licensure required (will accept pending DC licensure with proof of application) 5. Education: Not specified 6. EMR: Cerner experience preferred 7. Shifts/Call: 08:00-18:30 4x10's Days; no on-call requirement; weekends included in rotation (every 5th or 6th weekend); holiday coverage as needed 8. RTO: No RTO permitted during first two weeks; no more than 7 days per 13-week contract; must be included at time of submittal 9. Returning Staff: Must have been separated for minimum of one year before returning as traveler 10. Travel/Local: 60-mile radius; locals accepted at lower rate; completion of at least 1 travel assignment required 11. Background: Not specified 12. Modules: Facility-specific Sitel modules must be completed within one week of signing contract; billable up to 4 hours, average 12 hours; completed pre-start and potentially during orientation 13.
