Job Description
Epic Travel Staffing is hiring a Travel – Social Worker Case Manager
Shift: Days / 8a-5p, 8×5, 40hrs per week
Length: 13 Weeks
Requirements:
- 2 years acute care hospital – OR – 1 year acute + 1 year of Hospice, HH or other work with elderly
- Skills required: EPIC experience, Drug & Alcohol Screens (SBIRT), Mental Health Evals (SAFE-T), Capacity Evals, Behavior Plans, BH referrals, Provide or Assist with Tele Pysch Evals, Discharge planning, end of life conversations, Single Bed Certs/ITA process, difficult placements, prior experience in home health, hospice, SNF’s, Medicaid/Medicare, insurance authorizations.
- BLS
- MSW & LICSW or LSWAIC (only if they do not need supervision by MHS)
Other Details:
- EMR: EPIC
- Will consider first time traveler
- Patient Types: Medical Surgical, ICU, Mother Baby, Labor and Delivery, ER coverage on weekend
