Job Description
Epic Travel Staffing is hiring a Travel – Travel – Social Worker
Shift: Variable Days / 7:30a-4p / 8a-4:30p, 8×5, 40 hrs per week, Monday-Friday,Every 4th Weekend, rotating holidays
Length: 13 weeks
Requirements:
- 3 years Acute Care Hospital Discharge Planning Experience
- Cerner experience
- WellSky experience
- Social Worker with Case Management experience in an acute care environment and in Transitions of Care/Discharge Planning
- NYS LCSW or LMSW
- BLS
- RoundTrip/Parachute/Payor Portals experience preferred
Other Details:
- Internal floating within the Case Management Team required to ensure coverage of all inpatient & outpatient coverage areas
