Experience: 2 years required in inpatient medical social work experience, with biopsychosocial assessment, assisting patients navigate the DCF process, collaborative care, care coordination with home care and equipment following discharge, experience in rehab and long term care referrals, knowledge of community programs and protective services. MUST have MEDICAL INPATIENT EXPERIENCE. 2. Cases: Not specified 3. Certifications: BLS, MSW required 4. License: LMSW, MD State License required (compact license or MD license accepted) 5.
