Requirements

- 1+ years of experience
- Key Requirements Summary: 1. Experience: 1 year required 2. Cases: Extremities, Abdomen/GE, Spine/Pelvis, Chest/Thorax, Musculoskeletal, Lower Extremities, Upper Extremities, Surgery/C-Arm required 3. Certifications: ARRT R, BLS, Fluoro Permit required 4. License: Oregon state license required (can submit with pending unless specified in job title) 5. Education: Not specified 6. EMR: Epic required 7. Shifts/Call: Block schedule; 3 call shifts per month; weekends required (Fri-Sun 5pm-5:30am) 8. RTO: 6 days total or 3 consecutive days auto-approved; must commit to working 3 of 5 holidays; must request upfront during submittal 9. Returning Staff: 3 months between permanent and travel; must include previous Providence work history 10. Travel/Local: 50-mile radius rule; locals accepted at lower rate; must have vehicle 11. Background: Any background hits must be sent for approval prior to submission 12. Modules: Non-billable, average 16 hours; completed during orientation and annually; over 20 hours requires approval 13. Travel Experience: Not specified as required 14. Scrubs: Any color (OR/Cath Lab provided by facility) 15. Flu Vaccine Requirements: Unknown 16.
