- Key Requirements Summary: 1. Experience: 1 year required for submission 2. Cases: Extremities, Abdomen/GE, Spine/Pelvis, Chest/Thorax, Musculoskeletal, Lower Extremities, Upper Extremities, Surgery/C-Arm required; Head, Fluoroscopy Studies preferred 3. Certifications: ARRT R, BLS, Fluoro Permit required 4. License: Oregon state license required (can submit with pending) 5. Education: Not specified 6. EMR: Epic experience required 7. Shifts/Call: 3 call shifts per month required; Mon-Thurs 11pm-9:30am; block schedules required 8. RTO: 6 days total or 3 consecutive days auto-approved; must commit to working 3 of 5 holidays; must request all RTO upfront during submittal 9. Returning Staff: 3 months between permanent and travel; must disclose previous Providence facility employment 10. Travel/Local: 50-mile radius rule; locals accepted at lower rate; must have vehicle 11. Background: Any background hits must be sent for approval prior to submission 12. Modules: 16 hours average, non-billable; completed during orientation and annually; self-reported via Workramp transcripts 13. Travel Experience: Not specified as required 14.
