Requirements

- Key Requirements Summary: 1. Experience: 1 year minimum, recent experience required 2. Cases: Geriatric cases; Home Health ONLY 3. Certifications: BLS required 4. License: License in-hand required with submission 5. Education: Not specified 6. EMR: HCHB (Homecare Homebase) experience REQUIRED; OASIS experience REQUIRED for staff completing SOC (Start of Care) 7. Shifts/Call: On-call rotation may be required; 30 visits weekly for 40-hour guarantee; all charting must be completed by 9a the following morning 8. RTO: Must be submitted at the time of application 9. Returning Staff: Former permanent employees must be separated for minimum 6 months before eligible to return as traveler; must be approved by HR 10. Travel/Local: 60-mile radius from hub/office; locals accepted at travel rate; active driver's license and personal transportation required 11. Background: Not specified 12.
