Requirements

Certifications: BLS required; specialty-relevant certification required for Allied positions at submission 4. License: Oregon RN license not required at submission (pending okay); Oregon CNA license must be in hand at submission; copy of Nursys/CNA license (PDF format, dated within last 30 days) required 5. Education: BS, MS, or Doctorate in Physical Therapy required; graduate of program approved by Committee on Accreditation in Education of the American Physical Therapy Association 6. EMR: Epic charting experience required (hard stop requirement) 7. Shifts/Call: 4 day work week, 9 hour days, 36-40 hours per week; includes weekend rotation and possible holiday shift 8. RTO: Must be finalized at time of submittal; additional RTO requests after submittal may result in removal from consideration or contract cancellation; facility does not accommodate block scheduling 9. Returning Staff: Former permanent employees may not submit to travel positions at any Salem Health facilities for six months after employment termination date 10. Travel/Local: Travel experience required; candidates may float within hospital or to West Valley Hospital (15 minutes away); refusing to float may result in contract cancellation 11. Background: License/background hits require detailed written statement and reviewed case-by-case; travelers with hits ineligible for 90-day temp license waiver 12.
