Requirements

- Key Requirements Summary: 1. Experience: 1 year required 2. Cases: IV admixing experience of non-hazardous medications required 3. Certifications: CPHt, OR state license required 4. License: State license required and must be in hand (cannot submit pending unless specified in job title) 5. Education: Not specified 6. EMR: Epic experience required 7. Shifts/Call: 10-hour shifts 0730-1800, consecutive weekends required, no on-call, up to 26 weekend shifts per contract 8. RTO: 6 days total or 3 consecutive days auto-approved; must commit to working 3 of 5 holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day) 9. Returning Staff: 3 months required between permanent and travel assignments; must disclose previous Providence facility employment 10. Travel/Local: 50-mile radius rule; locals accepted at lower rate; must have vehicle for assignment 11. Background: Any background hits require approval prior to submission 12. Modules: Average 16 hours, non-billable; completed pre-start and annually; over 20 hours requires PM approval 13. Travel Experience: Required 14. Scrubs: Any color except OR/L&D which are provided 15. Flu Vaccine Requirements: Unknown 16. Special Equipment: Pyxis required Critical: Must have Pyxis and Epic experience. Two references covering 1 year within last 3 years required. May float between sister facilities.
