- Key Requirements Summary: 1. Experience: 5 years required; 3+ years in managed care environment required; 1 year experience unless specified in title/description 2. Cases: Managed care including Prior Authorizations, Audits, Appeals, ALJ Court hearings; Medication Therapy Management (MTM) program; population-based disease management; specialty pharmaceutical acquisition and care coordination 3. Certifications: Board Certified preferred; PharmD required; PGY-1 Residency training required 4. License: OR, WA, or CA Pharmacist License required; OR license can submit with pending unless specified in job title 5. Education: PharmD required; PGY-1 Residency training with 1 year experience; Bachelor's or MS Degree in pharmacy considered with 5+ years managed care experience; PGY-2 Residency preferred 6. EMR: EPIC required 7. Shifts/Call: Remote M-F; rare weekends required a few times per year 8. RTO: 6 days total or 3 consecutive days auto-approved; must commit to working 3 of 5 holidays; must request all RTO upfront during submittal 9. Returning Staff: 3 months between permanent and travel; must disclose previous Providence facility work history 10. Travel/Local: 50-mile radius rule; locals accepted at lower rate; must have vehicle for assignment 11. Background: Any background hits must be sent for approval prior to submission 12.
