Requirements

- Key Requirements Summary: 1. Experience: 1 year required unless specified in title/description 2. Cases: Must have outpatient infusion experience 3. Certifications: Chemo/Hazardous Drug, Oregon Pharmacist License required 4. License: OR license required, can submit with it pending 5. Education: Not specified 6. EMR: Epic required 7. Shifts/Call: Block schedules required, no on-call, no weekends required; patient ratios 1:10 days and weekends 8. RTO: 6 days total or 3 consecutive days auto-approved; must commit to working 3 of 5 holidays; all RTO must be requested upfront during submittal 9. Returning Staff: 3 months between permanent and travel; must disclose previous Providence facility work history 10. Travel/Local: 50-mile radius rule; locals accepted at lower rate 11. Background: Any background hits require approval submission 12. Modules: Average 16 hours, non-billable; completed pre-start and annually; over 20 hours requires PM approval; self-reported completion recorded in Workramp transcripts 13.
