Requirements
- Required certification: Pharmacist
- 1+ years of experience
- Key Requirements Summary: 1. Experience: 1 year required; infusion center experience required (will consider inpatient with oncology experience) 2. Cases: Outpatient oncology; chemotherapy, IV piggyback, IV push medications, packaging antineoplastic agents 3. Certifications: Chemo/Hazardous Drug and RPh required; must obtain all required certifications prior to start 4. License: VA state license or Compact required with submission; current Nursys report required 5. Education: B.S. Pharm. or Pharm.D required; pharmacy residency from accredited ASHP institution preferred 6. EMR: Epic preferred 7. Shifts/Call: Rotating 8am-4:30pm Monday-Friday (may start at 7am); weekend and holiday rotation; no on-call required 8. RTO: Need to add week at end of assignment if requesting more than 7 days; OR/CST unit only allows 1 holiday request; at manager's discretion 9. Returning Staff: Must be gone 1 year before returning as traveler 10. Travel/Local: No radius rule; locals accepted at same rate; may float to affiliate hospitals 11. Background: Nursys only 12. Modules: 8 hours average, completed pre-start via WorkRamp, non-billable 13. Travel Experience: First-time travelers accepted 14. Scrubs: Not mentioned 15. Flu Vaccine Requirements: Required with no exemptions 16. Special Equipment: Hazardous and non-hazardous medications Critical: Must include middle initial at submission. Required experience in intravenous preparation and pharmacotherapy management for gastrointestinal, hematologic, immunologic, neurologic, oncologic, and rheumatologic conditions.
