- Key Requirements Summary: 1. Experience: Minimum 3 years required (18 months mentioned in general policy but job specifically requires 3 years) 2. Cases: General OR and Trauma required; specific case experience required including Bariatric (Laparoscopic assisted), General Surgery (Appendectomy, Cholecystectomy, Colon resection, Hernia repair, Mastectomy, Thyroidectomy, Whipple), Neurology (Cervical fusion, Craniotomy, Laminectomy/discectomy, Lumbar fusion), Orthopedic (Arthroscopy, Carpal Tunnel, fracture reductions), Plastic Surgery (Free Flap - Microvascular), Transplant (Organ donation), Urology (Cystoscopy, Nephrectomy, Prostatectomy), Trauma (Amputations, Gunshot/stab wounds, MVA) 3. Certifications: BLS required, CNOR preferred; AHA and Red Cross only, RQI cards not accepted 4. License: Active IL license in hand required at submission 5. Education: Not specified 6. EMR: Cerner experience preferred 7. Shifts/Call: On-call required 1/week, weekend coverage 1 per month, ALT shifts 1045-2115/1245-2315 8. RTO: Auto-approve 5 days non-holiday RTO or less, must work 2 out of 3 major holidays 9. Returning Staff: Former PERM or PRN employees must be away 12 months before applying as traveler 10. Travel/Local: Must live 50+ miles from facility for OR RNs 11. Background: Background policy noted, must seek approval prior to offer 12. Modules: Non-billable up to 16 hours, completed pre-start by 4pm CST Monday, sent Friday before start 13. Travel Experience: Required 14. Scrubs: Provided for this role 15. Flu Vaccine Requirements: Required with Medical/Religious exemptions only 16. Special Equipment: Electrosurgical units, Fiber optic light sources/cords, Hana tables required Critical: Level I trauma experience required. SSN and DOB required at submittal. Lab appointment, Docusigns and Badge Photo must be completed within 1 week of signing contract. First-timers not accepted.
