- Key Requirements Summary: 1. Experience: Minimum 3 years required (18 months minimum mentioned in compliance notes) 2. Cases: General OR and Trauma required; specific case experience required including Appendectomy, Cholecystectomy, Colon resection, Hernia repair, Laparoscopic procedures, Mastectomy, Thyroidectomy, Whipple, Cervical fusion, Craniotomy, Laminectomy, Lumbar fusion, Arthroscopy, Carpal Tunnel release, fracture reductions, Free Flap Microvascular, Organ donation, Cystoscopy, Nephrectomy, Prostatectomy, Amputations, Gunshot/stab wounds, MVA trauma 3. Certifications: BLS required, CNOR preferred; AHA and Red Cross only, RQI cards not accepted 4. License: Active IL license required in hand at submission 5. Education: Not specified 6. EMR: Cerner experience preferred/required 7. Shifts/Call: On-call 1/week required; weekend coverage 1 per month 8. RTO: Auto-approve 5 days non-holiday RTO or less, anything more requires facility approval; must work 2 out of 3 major holidays 9. Returning Staff: Memorial PERM or PRN employees must be away 12 months before applying as traveler 10. Travel/Local: OR RNs must live 50+ miles from facility 11. Background: Must disclose background policy prior to submission for approval before offer 12. Modules: Non-billable up to 16 hours, then billable; completed pre-start by 4pm CST on first day; sent Friday before start; strict deadlines enforced 13. Travel Experience: Required 14. Scrubs: Provided for this role 15. Flu Vaccine Requirements: Required with medical/religious exemptions only 16. Special Equipment: Electrosurgical units, Fiber optic light sources/cords, Hana tables Critical: Level I trauma experience required. SSN and DOB required at submittal. Lab appointment, Docusigns and Badge Photo must be completed within 1 week of signing contract.
