Epic Travel Staffing is hiring an Interim – Nurse Supervisor – Case Management

Shift: Non-exempt, Days, 9a-5:30p, Mon-Fri, 8×5, 40 hrs per week
Duration: 14 weeks

Requirements:

• New York RN License
• 7 years of experience, 5 of which in a supervisory role
• 2 years of experience in discharge planning and utilization review
• BLS
• CCM Certification
• PRI Certification
• Bachelor’s degree in Nursing from an accredited Nursing Program
• Preferred: Master’s degree in Nursing

Other Details:

• Transitional care division within Care Management; works with the transitional care team to reduce 30-day readmissions and serve the high-risk post-discharge population from inpatient med/surg units and the Adult Emergency Department
• Provides Associate Director level supervision to a Care Management Department
• Provides oversight and direction to Accountable Care Managers (RNs), Care Coordinators (CLWs), and clerical and ancillary staff
• Performs all duties of the Director of Nursing in their absence
• Skills required: program development, performance monitoring, record-keeping system development, care management, patient care improvement
