Requirements

- Key Requirements Summary: 1. Experience: 1 year required; must have recent patient care experience in last 3 months 2. Cases: Skilled home care experience required (will not accept private duty nursing only); General Medical Dx; High risk Dx: HF, COPD, MI, Pneumonia, Stroke; Post-op care 3. Certifications: BCLS required; must have OASIS training and experience 4. License: FL license or compact license required at time of submittal 5. Education: Not specified 6. EMR: Cerner charting experience preferred; BayCare Forms, BayCare Scheduling, Ace Timesheet documentation system 7. Shifts/Call: Home skilled visits 6-8 per day; Mon-Fri 8a-5p or 9a-6p; Thurs-Mon or Fri-Tues shifts; Alternating weekend shifts 8. RTO: BayCare does not allow any RTO in contracts 9. Returning Staff: Permanent employees must wait at least 1 year before returning; return travelers only allowed in ICU, MICU, CVICU, ED, PCU, and MS/Tele specialties; must have completed contract within last year since 7/1/2022 10. Travel/Local: 100-mile radius rule; traveler must live at least 100 miles from facility; locals accepted at same rate 11. Background: Not specified 12.
