Requirements
- Required certification: Home Health Care
- 1+ years of experience
- Active California RN license required before start
- Unit Notes:Profiles are sent directly to the unit need 2 sup refs from modality submitting to (charge accepted) at submittal; 1 w/in 12 mo. & 1 w/in 3 years worked Candidates may not have been directly employed (full time, part time, or PRN) by any facility associated with CommonSpirit, including CHI and/or Dignity within the past year. Candidates who were employed PRN through outside firms will be considered Department: Hospice RN - Service area is the greater Sacramento area. We service a 50 mile radius from 3400 Data Drive General Certification Requirements (BLS, etc.) BLS Other Certifications/Specialty certifications requirements? None Case load/# patients per day/consults per day Caseload=12-14; 4 visits a day or 2 SOCs a day EMR HCHB: Home Care Home Base Do you require InterQual criteria or Milliman experience No Is this position for UR, discharge planning, or both? N/A What areas do they cover: Hospice-home care Common types of patients & job duties Hospice patients in homes or facilities Experience or 'Must have' skills 1 year hospice experience Shifts & Scheduling?
