- Key Requirements Summary: 1. Experience: Minimum 1 year Home Health experience; 2 years overall clinical experience required 2. Cases: In-home Patient Care, Interdisciplinary Care Coordination, Plan of Care Development and Oversight, Geriatric Population 3. Certifications: BLS/CPR required; CHPN certification preferred; must be in hand at time of submission 4. License: Current state license as Registered Nurse required; will not accept hits on license 5. Education: Not specified 6. EMR: HCHB and OASIS experience required for all; if no HCHB training, traveler must complete at $250 upfront cost 7. Shifts/Call: Must be open to 1 night of call during the week; weekends as needed; on-call rotation M-F 8a-5p 8. RTO: RTO at time of submission only; NO RTO accepted after submission 9.
