- Key Requirements Summary: 1. Experience: 1-2 years as Registered Nurse in clinical care setting required; minimum 1 year hospice experience required, no exceptions 2. Cases: In-home Patient Care, Interdisciplinary Care Coordination, Plan of Care Development and Oversight, Geriatric Population, Homecare; 4-6 patients per day average; 6 points per day minimum, up to 8 points daily; 30 points per week minimum required 3. Certifications: BLS required; current CPR certification required; CHPN certification preferred; must be valid and in hand at time of submission 4. License: Current state license as Registered Nurse required; valid license in hand at time of submission; will NOT accept hits on license; current driver's license required 5. Education: Not specified 6. EMR: HCHB and Oasis experience required for all RN candidates; Homecare Homebase training experience required; if traveler lacks HCHB training, must complete at $250 cost paid upfront before starting 7. Shifts/Call: Must be open to 1 night of call during the week; weekends as needed; on-call rotation Monday-Friday 8a-5p 8. RTO: RTO at time of submission only; NO RTO accepted after submission 9.
