Adults, Geriatrics, Peds - EMR HomeCare HomeBase - Shifts & Scheduling (Do you allow self-scheduling? Block scheduling?): 8a-5p. On call rotation for evenings - Weekend Requirements: On call rotation - Holiday Requirements: Holiday coverage required - Scrub Color: Any solid color scrubs - Job Responsibilities: Initial assessment of client and family to determine home health needs - complete physical assessment & history of current & previous illness. Regularly re-evaluates client nursing needs. Uses health assessment data to determine nursing diagnosis. Initiates plan of care, makes necessary revisions as client status & needs change. Develop care plan/establish goals based on diagnosis & incorporate therapeutic, preventive, rehabilitative nursing actions. Include the client/family in planning process. Administers medications and treatments as prescribed by the physician. Provides health care instructions to the client as appropriate per assessment and plan of care. Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the client. Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate client care for assigned caseload. Prepares clinical notes and updates the primary physician when necessary and at least every sixty days. Communicates with the physician regarding the client s needs and reports any changes in the client s condition; obtains/receives physician s orders as required. Ensures that arrangements for equipment and other necessary items and services are available. Instructs, supervises and evaluates home health aide care provided Service 60 mile radius around Dickinson - provide care to many patients, homes, nursing homes, etc. Radius Rule: 50 miles AND candidates may not be full time, part time, or PRN with any facility associated with CommonSpirit, including CHI and/or Dignity within the past year RTO needed upfront - Additional RTO will not be approved, including after submissions.
