- Unit Notes:Med Sol completes a profile review and manager will conduct a full interview need 2 sup refs from modality submitting to (charge accepted) at submittal; 1 w/in 12 mo. & 1 w/in 3 years worked Candidates may not have been directly employed (full time, part time, or PRN) by any facility associated with CommonSpirit, including CHI and/or Dignity within the past year. Candidates who were employed PRN through outside firms will be considered. ***COVERAGE FOR LOA - NEED A CANDIDATE TO START ASAP*** Unit Inpatient Palliative Care (Manager- Joni Johnson) # consults per day 10-15 consults per day for the entire team. Required Certifications BLS, ELNEC preferred (end of life care certification) EMR Cerner Common diagnoses/Types of patients Any patient that meets the Palliative Care screening requirements in the hospital. Physician places order consultation for Palliative Team. RN carries case load, visits up to 5-6 New patients, 3-4 follow up visits per day. Specific Job Duties Related to this role? Palliative Care Experience a plus! Developing patient daily list, Review of Patient charts, assessing patient needs, Opening Goals of care discussions, Education on disease process, Advance Directives and POLSTs, Holds family meetings as needed, End of Life discussions/support. Attends daily Rounds. Experience or 'Must have' skills Must have 2-5 years of recent experience in palliative and/or hospice setting, ICU and or oncology Experience a plus. Strong Acute Care, clinical thinking and communication skills necessary. Flexibility and strong organizational skills a must have. Desire to serve others - patient's families, team members a must. Charge Nurse/House Supervisor (Is the charge nurse required to take a patient assignment?) Palliative Care RN Coordinator: No hands on patient care. Shifts & Scheduling (Do you allow self-scheduling? Block scheduling?) 8hr shifts, 0730-1600, 0800 - 1630. Weekend Requirements Up to every other weekend Holiday Requirements Rotate in with core staff Call Requirement No Call Requirements Floating Requirements No floating, will visit many units, as patients are served throughout Medical Center units and floors. Scrub Color/Dress Code Business Casual, comfortable shoes, White lab jacket, long. Unit/Department specific orientation 3-4 Days with PC RN Coordinator/Physicians Additional Notes Faith based Organization. Palliative care team currently consists of 2 Physicians, 1 NP, 3 RN's, 3 Supplemental RN's. Palliative Care daily census 55 patient, with average of 15-17 new patient's per day. Cancellation Policy:Hospital can cancel up to 4 shifts per 13-week assignment, 3 shifts for assignments that are less than 13 weeks Charting System:Cerner BSN Required:False License to Submit:Yes Trauma Level:2 Care Setting:Acute Division:Nursing Shift Notes:8s; 0730 or 0800 start time, EOW, First to Float Required Certifications:BLS Certification
