- Key Requirements Summary: 1. Experience: 1 year required for submission 2. Cases: Cath Lab & Angio with scrub experience; complex cardiac interventions including OCT, IVUS, Rotoblator, Shockwave, drug coated balloons, CTO equipment, impella insertion, IABP, pericardiocentesis 3. Certifications: BLS, ACLS, ARRT-R, OBMI (Oregon state license) required 4. License: Oregon State License or proof of OR License application/fingerprints required with submission 5. Education: Not specified 6. EMR: Epic experience required for RNs; EPIC Test Out Module required (mandatory 8-hour training if not passed) 7. Shifts/Call: 4x10 Days Mon-Fri 0700-1730; one weeknight per week and one weekend every 3-4 weeks call required 8. RTO: No RTO within first 2 weeks; maximum 7 days RTO; must include all RTO with submission 9. Returning Staff: 1 year separation rule required 10. Travel/Local: Locals NOT accepted - cannot live within 50 miles of ANY Legacy facility 11. Background: NO felony hits; must include traveler statement and WA with submission 12. Modules: Pre-start modules 4-8 hours non-billable; orientation modules billable or attributed to NBO if completed in first 8 hours 13. Travel Experience: Not specified as required 14.
