- Key Requirements Summary: 1. Experience: 1 year required 2. Cases: Experienced in Cath Lab & Angio with scrub experience preferred, will accept CVT with Scrub experience in all service lines - Cardiac, Neuro, etc 3. Certifications: BLS, ACLS, RCIS, OBMI (Oregon state license) required 4. License: Oregon State License or proof of OR License application/fingerprints required with submission 5. Education: Not specified 6. EMR: Epic experience required for RNs; must complete EPIC Test Out Module 7. Shifts/Call: 4x10 Days Mon-Fri 0700-1730; one weeknight per week and one weekend every 3-4 weeks call 8. RTO: No RTO in first 2 weeks; Max 7 days of RTO; must include all RTO with submission 9. Returning Staff: 1 year separation rule 10. Travel/Local: Locals NOT accepted - cannot live within 50 miles of ANY Legacy facility 11. Background: NO Felony hits; must include traveler statement and WA with submission 12. Modules: Pre-start modules average 4-8 hours non-billable; modules during orientation billable or attributed to NBO if completed in first 8 hours 13. Travel Experience: Not specified as required 14. Scrubs: X-Ray Tech/OR/IR/Cath Lab/PACU - provided by hospital 15. Flu Vaccine Requirements: Unknown 16.
