Requirements

- Active California RN license required before start
- Key Requirements Summary: 1. Experience: 2 years required for submission; 5+ years of RN Case Management experience required; Must have Inpatient Acute Case Management (Case Manager Hospital experience) 2. Cases: Case Management for acute care patients, care coordination, care transitions planning 3. Certifications: CA RN LICENSE, BLS required; all certifications must be AHA and in hand at time of submittal; copies required 4. License: CA RN License; valid state license required at time of submission 5. Education: Not specified 6. EMR: Epic experience preferred; specific charting experience required 7. Shifts/Call: 5/8s schedule, every other weekend, no call required 8. RTO: Only one holiday will be approved 9. Returning Staff: Must be gone 1 year to return as traveler 10.
