Requirements

- 1+ years of experience
- Active California RN license required before start
- Key Requirements Summary: 1. Experience: 5 years required; 2 years required for submission 2. Cases: Case Manager role cross-trained on all units; acute care experience required; direct hospital-based discharge planning experience; inpatient and Emergency Department case management experience 3. Certifications: RN License, BLS, CCM/ACM required; all certs must be AHA and in hand at time of submittal 4. License: Valid State License required at time of submission 5. Education: Not specified 6. EMR: Epic experience preferred 7. Shifts/Call: Variable start times between 7am and 3:30pm; every other weekend; floating to various units as needed 8. RTO: Only one holiday will be approved 9.
