Shift: Days / 8's or 10's, 40hrs per week

Length: 13 weeks

Requirements:

MA License
2 years of Case Management experience
6-months discharge planning Case Management experience in an Acute Care hospital
BLS

Other Details:

This should ideally be a large organization/teaching hospital with 500 beds or more
