- Key Requirements Summary: 1. Experience: 2 years acute care Hospital Case Manager experience performing Care Coordination and Discharge planning required; hospital experience is a must 2. Cases: Care coordination, discharge planning, InterQual criteria, DRG knowledge required 3. Certifications: Not specified 4. License: CA state license required; pending license accepted but must be licensed upon start date 5. Education: Not specified 6.
