- Key Requirements Summary: 1. Experience: 2 years Acute Care Case Management/Discharge Planning required; must have hospital experience (behavioral health experience does not apply) 2. Cases: Case load up to 25 patients; patient ratios days 1:25, weekends 1:45 3. Certifications: Not specified 4. License: CA state license required; pending license accepted but must be licensed upon start date 5. Education: Not specified 6. EMR: Epic required 7. Shifts/Call: Weekend requirement to meet department needs (every other weekend); must commit to working 3 of 5 holidays (Thanksgiving, Christmas Eve, Christmas Day, NYE, NYD) 8.
