• VT license
• 5 years of experience
• Sterile Compounding Competency (finger tip and broth test) along with math test
• Staff Pharmacist competencies to include medication reconciliation, order verification, management of vancomycin dosing and labs, and renal dosing
• Successfully completed NAPLEX
• BLS
